The table format forces this structure: you can't fill in a row without all three columns. If someone can't name an owner and a date, the action item isn't real — it's a wish. Surface that ambiguity in the meeting rather than writing it down as if it's decided.

Common mistakes to avoid
Writing discussion summaries instead of decisions. "We discussed the timeline and everyone shared their thoughts" tells no one anything useful. "We agreed to push the launch by two weeks to March 15" is what matters. Document the outcome, not the conversation.
Assigning action items without deadlines. An action item without a date is a suggestion. If you don't know when something is due, pick a date in the meeting. "Soon" and "next week" are not acceptable entries in the due column.
Sending notes too late. Meeting notes sent 3 days after the meeting are nearly useless. Action items should be captured and shared within 2 hours, while the meeting is still fresh and before people have moved on to other things.
